Restorative Program

Patient Diagnosis

The Laurels’ Restorative Program is designed for those individuals requiring post-acute rehabilitation or experiencing physical or mental deterioration. The program focuses on improving or maintaining physical, mental and psychosocial functional abilities.

Program Design

The Laurels’ Restorative Program is initiated upon completion of the therapy screening protocol, indicating the need for assistance, and approval from the guest’s physician. A licensed therapist assesses the guest and establishes the appropriate specialized activity. The program is performed by a restorative aide specifically trained within that specialty under the supervision of the therapist. Ongoing records are maintained to monitor the guest’s progress.  The guest remains in the program until the desired outcome is achieved or the guest is unable to continue the activity. Our Restorative Program includes nine specialties:

  • Ambulation Program

  • Bed Mobility Program

  • Communication Program

  • Dining Program

  • Dressing/Grooming Program

  • Orthotic/Prosthetic Program

  • Range of Motion Program

  • Toileting Program

  • Transfer Program

The Laurels’ Restorative Program interdisciplinary team is under the supervision of a licensed physical or occupational therapist with active participation by the guests, their physicians and their families. The team may include professional nurses, restorative aides, nursing assistants, social services, dietary, recreation therapists and other medical professionals relevant to the program specialty. The team works together to reach a common goal and ensures:

  • A sharing of expertise and a coordination of activities

  • Active participation by our guests and their support persons

  • A holistic view of our guests’ needs

  • A comprehensive, consistent, non-fragmented approach to foster optimal outcome

  • A focus on problem solving and flexibility

  • A reduction of health care costs and improved outcomes

Program Protocol

Attending primary care physicians and/or surgeons are welcome to follow their patients during their stay. If that is not feasible, arrangements can be made to meet with and appoint one of the Laurel physicians to attend to the patient. Progress notes and discharge summaries are delivered to the referring physician on a regular basis.
